Chapter 30. Geriatric Evaluation and Management Units for Hospitalized Patients
نویسندگان
چکیده
Inpatient care of elders with multiple co-morbidities requires close attention to the special needs and geriatric syndromes that arise in this vulnerable population. One strategy to address the risks of hospitalization is to provide care by a multidisciplinary team in a dedicated geriatric unit, principally Geriatric Evaluation and Management (GEM) Units. This model of care, in many ways similar to coordinated stroke units, may improve mortality and other clinically relevant outcomes compared with outcomes achieved on a general medical ward. An alternative strategy, reviewed elsewhere in this Report, is the comprehensive geriatric consultation service, analogous to a typical medical consultation team (see Chapter 29). In a GEM unit, a multidisciplinary team provides comprehensive geriatric assessment, detailed treatment plans, and attention to the rehabilitative needs of older patients. A typical team is composed of a geriatrician, clinical nurse specialist, social worker, and specialists from such fields as occupational and physical therapy, nutrition, pharmacy, audiology, and psychology. GEM units are typically separate hospital wards that have been redesigned to facilitate care of the geriatric patient. Multidisciplinary team rounds and patient-centered team conferences are hallmarks of care on these units, which, in contrast to geriatric consultation services, have direct control over the implementation of team recommendations.

Outcomes of continuous process improvement of nutritional care program among geriatric units.
BACKGROUND Up to 65% of elderly patients are protein-calorie undernourished at admission or acquire nutritional deficits while hospitalized. The aims of this project were: (a) to assess the quality of care concerning nutrition among Belgian geriatric units, (b) to include more routinely nutritional assessments and interventions in comprehensive geriatric assessment, and (c) to assess the impact...
متن کاملAnticholinergic burden and cognitive function in a large German cohort of hospitalized geriatric patients
PURPOSE Previous studies suggest an association between use of anticholinergic drugs in elderly patients and cognitive impairment. However, there are still limited data on the association of anticholinergic drug use and cognitive impairment as well as contribution of individual drugs to anticholinergic load using large, well-documented patient cohorts treated in geriatric units from Europe. M...

Introduction: Overcoming the existing crisis in the hospitals without considering the patients needs and their satisfaction is impossible. The level of hospitalized patients satisfaction with different units is one of the important criterions for evaluation of the quality of hospital services. Methods: The present study was carried out by a cross-sectional method during the spring of 2002 for e...
متن کاملChapter 16: Disorders of Serum Sodium Concentration in the Elderly Patient
Disorders of serum sodium concentration are the most common electrolyte abnormalities seen in the geriatric population.1 Furthermore, the development of serum sodium abnormalities is associated with increased morbidity and mortality in affected patients.2 Often, however, the severity of the primary process contributing to the development of the abnormal serum sodium is responsible for the unsat...
متن کاملPrediction of hospital events based on the severity of illness
We read with interest the article ‘‘Medical adverse events in elderly hospitalized patients: A prospective study,’’ by Szlejf et al. (1). Using predictive factors, the authors aimed to determine the frequency of adverse medical events in elderly patients who were admitted to an acute care geriatric unit and the correlation between adverse events and hospital mortality rates. They defined an adv...
